Do I need weight loss medication?

Medical Criteria for Weight Loss Medication

  • BMI of 30 or higher
  • BMI of 27 to 29.9 plus a medical problem like high blood pressure, type 2 diabetes, high cholesterol, joint pain, or sleep apnea.
  • Failed lifestyle changes: You tried diet and exercise for 3–6 months without losing at least 5% of your body weight.

If you're here reading this, it's likely you've already tried diet, exercise, intermittent fasting, and many lifestyle and behavior changes. Many times you did lose weight — but a few months later you regained it all back. This is called "yo-yo weight loss."

You will benefit from weight loss medication if:

✓  You have obesity or obesity-related conditions like diabetes or high blood pressure

✓  You are considering bariatric surgery

✓  You have post-pregnancy or post-menopausal weight gain

Which GLP-1 is the most effective in causing weight loss?

The most effective GLP-1 related medication for weight loss is Tirzepatide ( marketed as Zepbound for weight loss and Mounjaro for treatment of type 2 diabetes)

According to a new meta-analysis from the University of Georgia, Tirzepatide particularly at higher doses, provides the greatest weight reduction in adults without diabetes. The review is the first to compare efficacy of the three FDA-approved GLP-1 medications in non-diabetic patients.

1) Tirzepatide, a dual GIP/GLP-1 receptor agonist, taken as a weekly injection; currently shows the highest overall weight reduction. It helped patients lose more than 20% of their starting body weight. Tirzepatide’s dual mechanism of action helps maximize weight loss

2) Semaglutide (marketed as Wegovy) yields an average weight loss of 15%. It is a GLP-1 receptor agonist available as a weekly injection or daily oral tablet

3) Liraglutide ( marketed as Saxenda) was the least effective, yielding 8% weight loss

How do GLP-1 work?

Benefits of GLP-1

In addition to achieving significant weight loss, research has shown that GLP-1 drugs can reduce the risk of major cardiovascular events such as heart attack or stroke in people with Type 2 diabetes. These benefits appear to extend even to people without diabetes who have a history of cardiovascular disease. GLP-1s are also approved to treat moderate to severe obstructive sleep apnea and metabolic-associated steatohepatitis (MASH), which was previously known as fatty liver disease. Use of these medications is also associated with improvements in mental health and quality of life.

Will I gain the weight back after I stop the medication?

Graph depicting How weight returns when treatment with semaglutide stops

Many patients maintain weight loss a year after stopping Ozempic

In the most recent analysis by Bartelt who studied 20,274 patients who lost atleast 5 lbs while on Semaglutide, found that 17.7 % regained all the weight they had lost after 1 year of discontinuing the medication and the rest 56.2% remained around the same weight they were when they stopped the medication.

These findings contrast with those from a previous clinical trial (Wegovy, Novo Nordisk) that showed that two-thirds of patients on once weekly Semaglutide 2.4 mg regained their weight 1 year after having stopped the medication.

What are GLP-1 medications?

GLP-1 medications are prescription drugs that mimic a natural gut hormone - Glucagon-like peptide-1 (GLP-1) to stimulate insulin release, slow digestion, and signal fullness to the brain. If diet and exercise haven’t helped you reach your weight loss goals, adding GLP-1 medications may provide that extra push as these have shown impressive results in clinical trials. These are used for Type 2 diabetes and obesity treatment

How do the GLP 1 medications work?

GLP-1 receptor agonists work by mimicking the natural gut hormone, GLP-1 . They activate GLP-1 receptors in the brain to decrease hunger and increase satiety. They slow gastric emptying resulting in a feeling of fullness, reduce appetite, lower preference for high calorie foods, decrease food cravings and help to achieve better overall control of eating habits. They also help the pancreas release insulin when blood sugar levels are high

What are the side effects of GLP 1 medications?

Nausea is the most common adverse effect. Other side effects include vomiting, diarrhea, constipation, acid reflux, bloating and stomach pain. These effects are usually mild to moderate and often improve over time

How long does it take to lose weight on a GLP-1 drug?

GLP-1 medications usually begin to produce visible weight loss within the first 1–2 months, with the most weight loss occurring over 6–12 months and beyond. Clinical trials show average losses of roughly 15-19% of starting body weight with Semaglutide and around 20-21% with Tirzepatide over about 68–72 weeks when used at full therapeutic doses alongside diet and activity changes.

What happens when you stop GLP-1?

When you stop taking a GLP-1 medication the drug's biological effects wear off and your body loses the benefits and positive health impacts it had. Your appetite returns to its baseline, hunger and food cravings increase, gut motility normalizes and digestion returns to its usual pace. Metabolism slows down leading to weight regain, higher Blood Sugars and A1c levels. Improvements in blood pressure and cholesterol levels may reverse over time. You may feel gradual relief from side effects like nausea, constipation, acid reflux and bloating.

Which GLP 1 is best for weight loss?

Tirzepatide, particularly at higher doses, provides the greatest weight reduction in adults without diabetes. According to a new meta-analysis from the University of Georgia, Tirzepatide (Zepbound) for weight loss and Mounjaro for treatment of type 2 diabetes, helped patients lose more than 20% of their starting body weight. The review is the first to compare efficacy of the three FDA-approved GLP-1 medications in non-diabetic patients.

Semaglutide (Wegovy) and liraglutide (Saxenda) did lead to weight loss, but it was significantly less at 15% and 8%, respectively.

Tirzepatide’s dual mechanism of action helps maximize weight loss

Do I have to exercise & eat healthy foods while taking a GLP-1?

These medications are most effective when combined with healthy eating and regular physical activity. Research shows that lifestyle habits amplify the benefits of these drugs and help sustain results over time.

How do you manage side effects of GLP 1 medications?

Eat smaller meals. Stop eating before you feel completely full.

Avoid greasy or fatty, fried food which worsens nausea and acid reflux.

Drink plenty of water and increase fibre in your diet to help prevent constipation.

Your Provider may start you on a low dose and increase doses gradually

What should you avoid while taking GLP-1?

While taking a GLP-1 medication you should avoid fatty, fried foods, sugary beverages and energy drinks, processed carbohydrates, spicy foods and heavy alcohol use which can worsen nausea, bloating, and acid reflux. Avoid eating big portions, eating too fast and try to stay upright for an hour or two after eating your meals
